What is Diagnostic Nasal Endoscopy (DNE)?
Diagnostic Nasal Endoscopy (DNE) is a minimally invasive procedure used to examine the nasal passages and diagnose nasal diseases.
It involves using an endoscope, a tubular instrument with a camera and light, to visualize the nasal cavity. This procedure helps ENT specialists accurately diagnose conditions like sinusitis, nasal polyps, and tumors.
Types of Nasal Endoscopy Procedures
Diagnostic Nasal Endoscopy (DNE): Used for diagnosing nasal conditions.
Therapeutic Nasal Endoscopy: Used for treatment purposes.
How does DNE Work?
During a DNE, an ENT specialist inserts a nasal endoscope with a 0-degree or 30-degree telescope into the nasal cavity. This allows the doctor to see detailed images of the nasal passages and identify any abnormalities.
What telescopes do we use in DNE?
The tubes used for Nasal Endoscopy should be under 4mm.
Dr. Meghanadh and his team use 1.9mm and 2.7mm telescopes with angulations of 0 degrees and 30 degrees, depending on the situation.
Benefits of DNE
Accurate Diagnosis: Provides a clear view of the nasal passages, allowing precise diagnosis.
Minimally Invasive: Compared to other diagnostic methods, DNE is less invasive.
Quick Procedure: Takes only 4 to 5 minutes.
Who needs a Diagnostic Nasal Endoscopy?
Diagnosing Nosebleeds: Identifying the exact source of bleeding.
Evaluating Nasal Blockages: Determining causes such as turbinate hypertrophy, infections, or tumors.
Assessing Sinusitis: Identifying fungal infections or bacterial infections indicated by the color of pus.
DNE for Sinusitis
For the initial diagnosis of sinusitis, nasal endoscopy is used to assess the patient's condition. Here are some specific findings:
Fungus: If there is a fungal ball in the nose, it indicates fungal sinusitis, which can be confirmed with a CT scan.
Green Pus: Indicates a bacterial infection caused by Pseudomonas, requiring targeted antibiotics.
Yellow Pus: Indicates a bacterial infection caused by Staphylococcus, also requiring specific antibiotics.
Preparing for DNE
No specific preparation is needed. It's advisable not to blow your nose before the procedure to ensure the doctor can observe any pus or blockages.
Anesthesia for DNE
General anesthesia is not required. Local anesthesia (LA) with xylocaine nasal drops may be used if the patient is sensitive to touch or if deeper examination is needed due to tumor or any other reasons.
If the patient has a known allergy to xylocaine, then LA is skipped.
Cost of DNE
The cost typically ranges from 1000 to 2000 rupees.
Side Effects
Side effects are not caused by the diagnostic nasal endoscopy procedure itself but can rarely occur due to the topical use of xylocaine nasal drops for local anesthesia. These side effects affect about 1 in 30,000 patients and may include symptoms such as sweating, nausea, or low blood pressure. Despite their rarity, these side effects can be effectively managed in a hospital setting. Since xylocaine is applied topically and not injected, any side effects are likely to wear off within 3 to 4 hours.
Post-Procedure Care
Generally, no special care is needed after the procedure. This is the least invasive procedure so no care is required.
